Title: Playstation 4 Cover Request - Disney Infinity 2.0

my 3 year old son decided it would be fun to chew on the case for my Disney Infinity 2.0 game and I am hoping to get a scan of the cover for it. I have replaced the case easily but can not find the case insert anywhere, HELP!!

Yeesh. Glad my kid didn't take to chewing on stuff. He just wants to see me play DI 2.0 or to "Watch Dada Pay a Yink Game"...Hyrule Warriors.
