Bump, as I've also tried to find this.  I know eightcell posted it here:

Eightcell's been MIA for a while now. I don't have access to his scans or I would make on myself. It some point I'm going to by the game (if I can find the box and manual with it) but until then, I don't know anyone who has a copy to scan. Sorry

Eightcell has quite a few scans (check the threads the NES Cover Project and NES Covers *early stages* to see all the material he has access to. Unfortuntely, he never posted usable material that isn't already here, and in addition, he seems to have fallen off the face of the planet  . But in fairness to him, I recall talking to him a while back and mentioning he had been rather busy with work, so I imagine that's only worsened. Poor chap.
