Playstation Move Disc - PS3 - NTSC-U

Does anyone have and can scan the cardboard sleeve that the PS3 Move Game Demo Disc came in?
I know PS3 isn't a top priority on the site, but hopefully someone will throw a cover together if the scans appear.
