1: There are a few PAL games that were optimized at 50hz and so have a glitch or two at 60hz so for 100% flawless, you need a text switch and a 'hz' switch. With those in place, everything runs.
2: Multi region released stuff tends to have both languages on the cart and adjusts to the switch. JP only games stay JP text only. Streets Of Rage for instance, becomes Bare Knuckle and has JP text.
3: Yes
4: Not really. But my TV (Samsung) handles the signal better through the composite connection than the upscaled Scart one.
5: MCD no. Still needs a CDX (or similar) or multi-bios to work. I use a Megacart for that purpose. It boots the Mega CD to any region depending what button you hold down on start up. Also is a flashcart and allows flashing from cd's in the Mega CD itself, if your into that sort of thing. 32X is ok for some games but needs a similar 'hz' selector to the MD for proper compatibility.
Megacart :

RGB to Component converter :
http://www.lektropacks.co.uk/view_product_standard.php?department=239&product=91&sub1=239&show=239One little problem is that these dont have a sound input.
MK1 MD's and MS's use the same RGB scart lead. The converter does'nt respond well to the MS signal though, i think its to do with the resolution (dont quote me on that). SNES/Saturn/Neo GEo/Dreamcast/Jaguar look very nice through it though
