Not the best, I know. But there aren't a huge amount of quality scans to work from, being a "big box" game.

I don't think it's retail only; more like official industry releases.  There are WiiWare game covers on here, for instance.  Please correct me if I'm wrong :-X

Not that I know of. I just grabbed that PC-CD logo of one of the covers already on the site and made my own.

Here is the heretic Cover that you needed.  Its not going to be perfect but its better than not have one at all.  Should be on box.net in a few minutes.

with the way the heretic cover is it would end up stretching the skull on the front too much. I would have to remove many of the things on the cover to make it work resulting in wrecking the look of a book.  I can try and see what I can do tho.
