I've made up the following templates using adobe photoshop and saved them in paint shop format.  As yet, these are not uploaded but if enough people are interested, I could PM Snowcone and see if the templates can be added somehow. 

If you want to see how the templates look when a game is added, refer to the 3DO post in the DVD case section (here) for the DOS PC case of Star Wars Rebel Assault..

All of your templates look great and I would love to use them as the standard for creating PC game covers. Could you create a Mac template as well? Thanks!

 Best part about pc covers is all you need is a good scan, no custom work necessary.

Here's your mac template.  You'll have to wait for the PSD's as they are 600K so I cant attach here and the PSD's wont go up the upload script.

Finally bit the bullet and shelved Adobe Photdeluxe BE 1.0 and installed gimp.  Will miss the clone tool though.  Anyway, I have taken the time to edit the PSD's to include standard spines in White, Black, and a Default spine (The default is shown in the jpegs below).  These would come in useful for all those older PC games that either came in cardboard boxes or in CD Jewel Cases, and for customising if full covers cant be found.

And heres a sample of the MS-DOS case.  I have uploaded to coloured (or should that be colored) spine, however I have made a black one as per the following:
