PC - Indiana Jones and the Infernal Machine - NTSC
I just bought it used for the PC and it wants a cover to keep it warm here at night. :)  If you're talented at doing customs you may be able to use the art from the n64 cover http://www.thecoverproject.net/view.php?game_id=569
