The official Nintendo cases that hold up to 1 DS game and 1 GBA game can be found at
http://store.nintendo.com/. To get to the cases, go to Parts, Accessories, and Manuals -> Nintendo DS ->Original Style Nintendo DS ->Parts &
Accessories ->Finally, the fifth from the top is Game Card Case(s). You should be able to figure out how to buy them.

KaiserWAVE says about Europe’s cases:
“You can get them from the Nintendo Europe homepage. On the German page they're listed under "Consumer Service" and then in the Nintendo DS section as spareparts for 4,50€ for a pack of three… As far as I heard from other people right now Germany and Italy are the only European countries that sell them.”
