My brother has about eight of those controller boxes lying around. (The one that KaiserWave showed scans of.) If no one else can provide better scans for them, I'm sure I could scan one. Might be a few days, though.

Crap. Scratch that 'few days' thing. An hour on the road, and I realized that I forgot my scanner. The soonest I would be able to scan a box is the 30th. (Unless someone I know around here has a scanner.) So, if anyone has that box and can scan it before then, feel free to.

I am currently unaware if this fits inside a UGC, mostlikely not, but it would be a simple fix to make it into a double case if need be.  Currently unable to find the Power Plug adapter, found this manual in a random box though

You should see if it fit in a wide EA Genesis case, i think it would look sharp next to the other genesis games on the shelf.

Started this - but now i'm too tired to finish - need to reformat the back and make the spine wider - it fits snugly in a double wide DVD case (or a UMC i am guessing)

It's hard to tell from the preview, but that back font looks hard to read. If it looks fine on the real cover, ignore this.

Sorry to drag this out of the depths of the General Discussion, but I just discovered this thread and I think this project deserves to live on!

I wish I had box scans I could contribute, but I thought I'd at least try to tack a whack at the SNES controller box using the scans provided earlier in this discussion. I'm not experienced at making more than PC covers, so I'm doing my best at creating it for the vertical SNES template. I also tracked down better images of Yoshi and Mario from Mario RPG. Of course it would be ideal if someone had better quality scans, but I can at least make something until then. Here's a preview of the front so far:



If there's any chance of this project continuing, I'd personally love to see a cover for the green Gameboy Color (if it fits in a UGC, or whatever works). My original one I grew up playing with was stolen long ago, but today I just got a practically new one, which is pretty exciting! I'd also love so see one for GBA and GBA SP eventually. If anyone out there has the boxes to scan, I'll even take a shot at them myself.
