Image verification doesn't work anymore, but the secret to spam-free forums is to use question verification.

Simply ask the registrant what the name of the site is, and don't register them if they don't answer "The Cover Project".  Killed spam on my forums altogether, and most forum software packages include the option to do it nowadays.
