Fire Emblem and Sonic Advance 2 for GBA are not the correct sizes. They both seem to be too short on both ends.

Not really sure if you guys can fix this since it would probably require brand new scans, but I figured I'd at least mention it here.

The back cover to Castlevania Circle of the Moon is a tad fucked up. Could do with some cleaning, cutting, and resizing.

I think that they did that because the actual N64 boxes have the logo like that. Who knows?

Considering the rest of the cover is vertical, I'd doubt it, but as you said, who knows...

Pointing out mistakes for custom covers is almost a lost cause unless you directly contact the original designer. Don't waste your time here.
Also, there's no need to mention low res. covers on a "Point out mistakes to be fixed" list, we already know about it, we just need HQ scans and people willing to spare its time with the community.
