I'll scan my EUR cover which is near identical to USA one bit later this evening.

You can still access TCPs raw scans section through the WayBackMachine. There are no covers for any platform of the current generation though:

I actually found it through searching forums and ps4 cover threads.
Won't let me attach it says full or too large. Just search forums and you should find the ps4 cover thread for anyone else that needs it.
