I still vote #1.
As for #3 you lowlies (jk) can write what ever you want as the name as we the few selected of The Apostoles of Snowcone (I and few other Cover Admins) have power to manually approve all covers (meaning no cover gets added to the site unless we add it) and we override all names to suit the agreed format in naming.

Was it scan or ready polished cover?

If raw, nothing will ever be seen of it as upload is for ready polished covers.
Raws should be distributed by uploading to Mediafire or Box.net and then linking to the boards in suitable old or new topic.

If polished ready cover:
1: Not polished enough we want perfect covers as we want to preserve them quality for generations to follow.
2: Still being checked and if near perfect maybe polished further.

If not absolutely perfect I again suggest Mediafire or box.net route.
