PC Games - Win/DOS - US - Custom or Retail, doesn't matter

I didn't even notice that Big Box website was European. That a little disappointing, but it's still a good source of PC raws for our European users. If anyone wants to download one of the covers on the site, click on a cover then click on the "download papercraft" button in the upper right corner of the screen.
I started on a few of your requests this evening. The template you provided will work just fine. I'm removing references to Steam on the back covers and adding the PC banner to the front and spine. Would you also like accurate barcodes and ESRB ratings on the covers? And do you want the company logo or the windows logo on the spine? And one last question regarding Star Wars Jedi Knight: Dark Forces II (what a mouthful!) was that released on DVD or CD?

Appreciated! To answer your Q's...
ESRB rating is ok to add, don't really care either way.
Barcodes yes, add that please. I have a cuecat so just in case I need to scan them in the future.
For consistency I suppose just the Windows/DOS logo depending on the release on the spine is better.
Star Wars Jedi Knight: Dark Forces II was released in 1997 so I'm going to assume it is a CD-ROM.
