Raw scan request PS2 DVD remote

Does anyone have the yellow NTSC scan of the PS2 DVD remote?

I can not find a good pic of the back.

I am looking to make a cover for this but I don't think it will fit into a standard 14 mm dvd case. I have some of the fat multi disc dvd cases I was going to try to make a cover for but I still can not find a good scan of the DVD remote insert. (specifically the back part)
