Looking for this cover created in a DVD case format, either retail or custom would be amazing thanks.

found a custom one here:

stole the back for one using the retail front - didn't spend too long on it as I couldn't find anything remotely decent to work with - if anyone has a good scan i'll gladly update it. (although removing the logo was a pain in the ass)
