I know this is out in the forums somewhere because I had to print it out for my copy of the game.  Trying using the search function and use quotes around your search.  If you can't find it by this evening, I will upload the copy I have saved on my computer.

Well, nevermind.  It turns out the copy I'm using is one I made for myself using my sub par Photoshop skills and images I found through Google (funny how we forget these things).

So here is a crappy version that you can use until someone makes a real one.
