Arseen,  I found a few threads from quite a while back saying you would try and make one of these for someone.  I'm wondering if by chance you ever did, and do you have a box.net or anything you keep them?  If not, no worries, but much appreciated if you do have it somewhere.

I tried my hand at making a copy of this myself.  I know it's not perfect, but here's the jpg and the original PSD.  If anybody is feeling particularly artistic, do you think somebody might be able to help touch it up a bit?
