Just checked ebay and theuy have 2 CIBs for sale with that image and another with another alternate image... Way too expensive to buy though.

I could scan the label if you could use that to build the front.... No idea if the back is any different then the other releases.

looks like mobygames actually has some alright scans for it... maybe able to put together an ok cover from it.
