I needed this one and didn't like the quality of what I found. I applied Shenske's template to it as well.

I have a cleaned up one in my box

Also protip - Despeckle in photoshop is a good way to remove Moiré patterns, just a random tip for future clean ups. \

Thanks larryinc64 for the despeckle info, I currently use the Reduce Noise filter. I tried comparing both and they give similar results, Reduce Noise feels somehow more agressive though.
