A few different games have done this. Persona, Kartia, RPG maker, Brigandine to name a few. Granted those are mostly RPGs, but it seems some games just do. I bought Fade to Black when it was new all those years ago and wondered the same thing.

Good to know. I'm kind of curious about the double CD jewel case though. Normally when a game is only one disc, but comes in a two disc case, it's because it either had a demo disc with it (think Resident Evil Director's Cut) or it had a manual so large it wouldn't fit into a normal jewel case (think Gran Turismo). But I can't figure out the reasoning for Fade to Black. The insert says 1 disc, so no demo disc, but the manual isn't that large either. Are you sure there wasn't a second insert that I'm missing?
By the way, I've already transferred it into a proper double CD jewel case.

We had this quite alot in PAL regions. Rumour was that there weren't enough of the wide PAL cases early on, so a number oif games came in these (Assault Rigs for one). I don't believe that story though, as it happened way after the Playstation had been around for a while (Destruction Derby 2, Loaded, Re-Loaded for example). And it certainly wouldnt be the reason for it in the US.

Went to the flea market today and picked these up!
Guy asked what I wanted to offer for both, and I was hesitant to offer 20 since I didn't know if it'd offend him but the hesitation paid off when he told me 15 for both!
They're disgustingly dirty because apparently he kept it around his birds but not dirty enough to scare me away lol.
