Here's Corpse Killer.

Here are a few tips to get everything to fit right.

•   You are going to have a few folds to make.  I use a straight edge and run a knife lightly over the paper to make a crease. 
•   For the front cover, I recommend making the crease first and folding the paper over.  Cut the top, bottom and ride side off.  Leave the back side folded around.  This will make the front cover a little thicker (two pieces of paper thick) which will hold the cover in the jewel case better.  The jewel cases were meant to hold thick manuals, and a single piece of paper will be very loose.  Having the back side folded around will help.
•   For the back cover, I recommend using one of your Sega CD or Sega Saturn covers to line up the crease marks.  Using this template will make the covers about 1 mm narrower than an original cover.  If you line up the creases with an original cover, you will not notice the 1 mm when everything is in the case.
