Probably not the worst, but the original Teenage Mutant Ninja Turtles game for the NES always bothered me since they all are wearing red.

That's not really a mistake though.  The cover art used was the same as the cover of one of the original comics; in those, all four turtles did indeed wear red.  The only reason people believe it's wrong is because they were introduced to the series through the show, which changed them for the sake of making them easier identifiable.  The game itself seems to be based more on the comics than the show, really.

Obvious answer: Castlevania Dawn of Sorrow's value release for the DS. I still think it was a joke that accidentally got sent off to the printers.
