Apparently, Wily Wars was scheduled for a physical release here, but was moved as a Sega Channel Exclusive due to debugging problems

It's rated for mature audiences? The ESRB rating system had been well established at this point in time. It's just the repro cover.

I was gonna say I thought it was a repro cover, but haven't done enough research to say I was sure about it.

No, the ART is correct for the planned release but the BOX is repro. Basically, when the art was discovered there were no logos or rating but they were added by repro makers.
