Request: Nintendo GameCube Action Replay (last release Wii compatible version)

Can someone do a cover for me for the last release GameCube action replay?  This was the one that came in the blister pack so it never had a case or anything.  I would like the standard GameCube template and use the scans' I'm including to fill out the front and back info.
