In need of a custom dvd cover for Koudelka that can handle all four discs in one cover.

I can provide scans of the existing PS1 front/back cd cover if needed.  Would just need to know where to put them.

If possible I like the style similar to this first Silent Hill cover (http://www.thecoverproject.net/view.php?cover_id=10270) with the Playstation on the crease and along the top front.

Scanned in the inlay.  It is at the same link as before.  The other one was just an ad for another game coming out.

I have started the cover at (http://1drv.ms/1ChriEj) using one of the templates provided.  I just seem to need some help with the back cover and also there seem to be some gaps between some of the template boundaries and my edits that I cannot seem to get rid of.  Anyone that can help it would be greatly appreciated.
