SNES: The Legend of Zelda: A Link to the Past (Player's Choice)

Does anyone have a LoZ:ALttP Player's Choice Cover Art formatted for UGC? I found a version where someone stuck the player's choice ribbon on an original run of the game art, but the Player's Choice box art is quite different. I would love a copy of this cover in the same format as seen here (http://www.thecoverproject.net/view.php?cover_id=487). Any help is appreciated.

I found a couple scans through Google and I can finish making the cover later today.
