[Request] Adventures of Lolo 3 Retail Cart Label - NTSC

I'm looking for a replacement label that is close to the retail lolo 3 label to repair a bad label. It looks like the custom covers on this site have almost the exact same art as the NTSC label but I have no idea what the top (part folder over with game name) is supposed to look like (and can't find a picture online of this part of the cart).
