I have a copy of the magazine and it looks like it is alot of text and images from this magazine.
I cannot read the text either but it really looks like the ads in the magazine (which span across 8 pages throughout the magazine.)
I'll slap together some images and sketches and we'll discuss it this weekend or early part of next week (whenever you have free time to do so.)

Here you go - there was 1 word i couldn't read - replaced with ???????. Also, the postcode in new jersey in section 1 may not be correct.
Other than that - does it REALLY say (3rd line from the bottom) that Sega is a trademark of Nintendo of America?!!??!?!

2. All contestants must register in the age category for which they qualify as of June 14, 1995, the first official day of the Competition. Participants may be required to provide proof of age (driver's license, birth certificate, or other legal document) at any stage of the competion.
3. Contestants sign up at participating BLOCKBUSTER VIDEO™ stores. Time slots are limited and will be assigned on a first-come, first-served basis.
4. All contestants must check in with the store's designated BLOCKBUSTER VIDEO™ store representative al least 15 minutes before his/her assined time slot, or he/she mas be disqualified.
5. No practice time on competition game equipment will be permitted prior to competing.
6. Each contestant will be required to initial his/her score when recorded by the BLOCKBUSTER VIDEO™ storerepresentative. Each contestant's initials shall indicate his/her acceptance of and agreement with the score. Participants prize (Including cards) must be signed for as acknowledgement of receipt.
7. Business will continue to be conducted in the usual manner in all participating BBV stores during the Store Championship Competition. Blockbuster Videos, Inc. and sponsors are not responsible for (a) any technicla difficulties or interuptions due to acts of God, labor disputes, illness or other causes beyond Blockbuster Videos, Inc. or sponsor's control.
8. At their own discression, BBV store representatives and/or judges may immediately disqualify any participant for conduct deemed to be faulty, mischievious, harmful or injurious to themselves and/or others.
