So many NES shmups would fall into this category for me.  I love the things, but MAN do they test the limits of my patience.  But, as a result, they are also the most satisfying to beat.  Off the top of my head, the ones that just piss me off the most:

Sky Shark
Star Force
Image Fight (that third boss...Christ I hate that thing)
Adventures of Dino Riki (though I have managed to finish this one, thanks to egregious abuse of the continue code)
Zanac

Recently, the hardest game for me has been Shinobi for PS2. That game's friggin' difficult!

I always have trouble with Fire Emblem games (Awakening excluded. That one I beat.)

It always boils down to someone dying in a certain stage, which means resetting and trying again. I'll get to one particular stage where I swear luck is just as important as skill, if not more (Fire Emblem's RNG is such utter bullshit), and my luck will always run out. This has been the case for every Fire Emblem game I've played except Awakening. I was overly powerful in Awakening, though I played on Normal, which everyone else regards as easy mode.

Call of Duty 3 for x360 on veteran made me give up about halfway through the game. I couldn't beat one of the checkpoints. I had similar moments in Call of Duty 2 where I couldn't seem to make progress after many attempts, but I was so enamored with the game I persevered through it and eventually finished.

Cod3 felt like it wasn't as well made as Call of Duty 2 so it wasn't worth digging deep for when I hit the really brutal sections of the game.

It seems that no one here has the memories I have of Silver Surfer for NES. That game was ridiculous! It easily blows Battletoads or any other game out of the water. Only exception to this would be I Wanna Be The Guy.
