Megatron:
I use this:

been using it for about 6 months.  It does take some getting used to, in terms of lining everything up, but as long as you give it a good chop it really looks amazing.

Here are some done with this cutter

note, Kingdom Hearts and Superstar Saga were NOT done by this.  been meaning to go back and fix all my sloppy work from earlier

To view the above link you'll need to use zip code.
I used my Finnish one and it worked. :D

Seems to be same as La Pointe, Wisconsin, USA
Bear78:
Clear ruler and a exatco knife.  Not always perfect but its pretty close and gets the job done.
Seraph Man:
Metal Ruler and X-acto knife. I also print the covers with a 1px thin black border, to help line everything up, which is especially helpful when you've got white background covers.

And it works like a charm.  I don't print my covers to full size, either, so I have two sides to trim to make a 1/4" border around the whole cover, and this thing is a wonder.

This is what I use, except mine is ancient and made of Oak or something and cast iron instead of plastic and fiberboard.  Before that (and still for some things that are too large for the cutter) I use a large ruler/straight edge and a knife.  Both get the job done, just depends on how much you want to invest.
