N64 Yoshi's Story Cartridge Label

I'm looking for a US version of the cartridge label for Yoshi's Story, if anyone can help me out with that that would be great.

I got a nice and minty copy, but my scanner is currently somewhere, so the scan might be a little while.

Yeah, sorry, I was not able to hook up my scanner yet, it only works on Win XP and lower, and I use Win 7, so I need to dig out a older pc.
