N64 Yoshi's Story Cartridge Label

I'm looking for a US version of the cartridge label for Yoshi's Story, if anyone can help me out with that that would be great.

Yeah, sorry, I was not able to hook up my scanner yet, it only works on Win XP and lower, and I use Win 7, so I need to dig out a older pc.

I pulled and threw my US cart on my scanner (which isn't hooked up to any PC, as I scan directly to a USB flash drive), and this is the result:

That's pretty good but mine's somewhat different than that. The rating is black not white and the code along the bottom is NUS-NYSE-USA-1. So I guess I'll see which one Larry has first.

I have the same label type has TSOL. The Rev you have seems to be the Players Choice version.

Maybe I can use TSOL's and mine as a reference to recreate mine.
