Anyone have Turtles in Time label?

Does anyone have a label for Teenage Mutant Ninja Turtles IV: Turtles in Time? SNES - NTSC
I have been scouring the forums a bit and haven't been able to find a working link. It would be greatly appreciated.

And I omly have PAL cart somewhere.

I'm virtually 100% sure we don't have it.
I tend to store stuff, and I don't have that, nor can I remember ever seeing one here.
NPES/Nintanbox draws blank too.
