PSX Mortal Kombat 3 Jewel Case

Is there anyone who has the extremely limited jewel case version of Mortal Kombat 3 for the original Playstation?  I'm looking for a scan or custom cover of the front and back artwork, as all I have is the long box version of the game.  Any help is greatly appreciated :-)
