Why are some of the covers "image not available" while other ones can be viewed and downloaded just fine?

that means that no one uploaded a cover

When you search a title, it will show you the title even when there are no covers available. Sometimes, for titles that have had covers rejected, the title will appear in the visible list of games even when there are no covers.

The names never get delisted. They stay there, patiently waiting for someone to upload a cover to that spot. If the name wasn't there, no one would be able to upload a cover under that game name.
