Title: Request Form - Retail and Custom Covers for NES and SNES

The Wiki request page still isn't working.

NES Custom Cover:
I'm looking for a Japanese Super Mario Bros. 2 (Famicom Disk System) Retail cover that will fit a Universal Game Case. If it's possible for it to have a cover, spine, and back text in Japanese .... that would be awesome!
(I have a FDS SMB2 in a NES cartridge. So looking to complete my cover collection in style!)

SNES horizontal Retail Covers:
NBA Jam (USA NTSC version)
NBA Jam: Tournament Edition (USA NTSC version)
Tecmo Super Bowl III: Final Edition (USA NTSC version)
