Alright, I've done a fair amount of digging, and even attempted making this cover myself, but I figured I'd ask to see if maybe someone more experienced with the covers can do a better job, or to see if someone has access to an actual box that can be scanned.
I have a copy of a Japan-only Gameboy game called "X" or "Ekkusu". I only have a cartridge. The best images of the box that I have found are taken from an eBay auction. Wikipedia also has a low resolution image of the front cover. I tried correcting the colors and putting the case design together, but I'm not really happy with the results so far.
I would like to put the game in a DS-style case using Shenske's Gameboy Template (silver spine).
The game doesn't seem to be very common, so I think it's unlikely that someone has a copy they can scan. Is it possible to use the eBay images and making something half-way decent?

The images you found may work well enough. I'm making another attempt at the case, but I don't have time to finish it this evening. Hopefully tomorrow I can post the results. It's not perfect, but it'll do.

Alright, so I put together a cover. I'm not sure it would pass quality control on the site, but I'm happy with it for my own collection. I adjusted it to fit some of the North American style (Nintendo logo at the bottom left and on the spine). I removed some of the Japan-only markings.
The dimensions of the back are a bit odd, but I don't really have another solution in this case.
This required a fairly extensive amount of Photoshop work. I left some of the work up to the content-aware tools (most notably the gray background). The original image had a price sticker over a portion of the image on the front. I rebuilt that portion of the case using a low res image on Wikipedia and some meticulous clone-stamp work. I'm actually rather happy with the rebuilt image.
