Title: Atari 5200 Template

Does anyone have a template for the Atari 5200?  I'm specifically looking for the following style.

To be more specific, I'm trying to convert the following cover into the above style.

I went ahead and put this together, but it would probably be a little cleaner starting with a proper template.  So I'm still looking for the template if anyone has one.  Thanks.
