Title: Request - N64- Earthworm Jim 3D - NA - Label

Hey guys, just seeing if anyone could help with the North American Nintendo 64 Earthworm Jim 3-D label? Retail or custom works as long as the custom is fairly close to the retail!  Thanks a bunch guys!
