The front could use more work, My 3DS template has HQ Old ESRB and Super Gameboy logos.
The copyright on the back looks wrong, and the Sku looks odd.

I am adding more covers to this list. But i have a question. I am simply editing the original post and adding the other covers. Is that the correct way to do that? Should i also be replying to the post when i add more? I will be adding about 20 more Game Boy covers over the next week. All of them i have to build from scratch based upon any low res scans i can find. I am using the Smart Blur feature of photoshop to try and improve the front cover quality. Any other suggestions?

I will post a list tomorrow. I was going to use your 3DS template but i would have to modify it to the dimensions of the POLY cases i am using. I am using the existing Game Boy Templates in the Template section as they size easily since the spine is the same size for both cases.
BTW. Just added Lemmings.

Could I beg someone to turn that Golf cover to a DS case format? I'd do it myself but my skills are so bad the original's would delete themselves out of shame ;)
Wave Race would also be appreciated, but I already have a cover (non-retail art, lo-res back) for it so not a priority.

No, i don't but maybe i'll try and work on getting a box and scanning it after i'm done with making the Game Boy covers on my list.

I found it after you mentioned it. But I didn't know it was there before i made it. It would help if you had a thread with all of your covers so they would come up in the forum search.

Added WWF Attitude. Had to make from scratch. It doesn't match the retail release and i couldn't fit the "only for Game Boy Color" on the front since i can't find appropriate artwork for the front without the Aklaim sports logo. If someone can either a) remove the log or B) make a new front i will change it for you. Thanks.

This is a quick custom cover. I used the art that was already done for the SNES covers and added GameBoy Screen shots.

I added Home Alone 2 Lost in New York for GB. Its based on the NES cover with GameBoy Screenshots. This should be considered a place holder until a regular cover is done.
