Yep, I actually just found it and Clayfighter among few other N64 covers on my other computer.

thanks to all that have helped so far! I'm currently printing these covers for 10 cents a pop at my university... :P
I'm in the works of modding genesis cases with 8.5 x 11 paper, for those of us who don't enjoy the legal type of printing LOL
I already set a thread about that, but hope to send some photos of the case idea to the gallery soon! This is definitely helping the cause :)

If you have a vertical Mortal Kombat 4 cover, that would mean a link should exist somewhere to the computer version of this cover right ?
I m planning on doing this one based on the horizontal release found on the website but if someone did the vertical conversion already, I just want to avoid doing covers that were already released... ;D

I think he meant that he has the box he can scan.
I don't think I have ever seen Vertical cover here for MK4.

There was a large price tag sticker over the UPC code that caused some minor damage in the process of removing it, so I'll add this to my list of eventual replacements to pick up, but the current scans should suffice.

Yea havent been home but it appears someone was preety eager to work on it after I already mentioned my mine was finished.
For future reference and for the sake of cuortesy, read theit thread and call it when youre filling request so that time isn't wasted.

Yeah, and I edited the spine, and improved it by making it "more green".
